SPF/SIG COORDINATOR

Salary Grade: 5
Reports to: Community Education and Prevention Supervisor
Job Group: Professional

GENERAL EXPECTATIONS:

ESSENTIAL JOB FUNCTIONS:

  • Promote destigmatization of the disease of addiction
  • Coordinate services with referral sources and other community resources
  • Maintain professional, personal, emotional balance through continuous self-examination,  supervision and feedback.
  • Serve as a central contact person with public and private organizations in the partnership or coalition that need services.
  • Establish working relationships with coalition members/agencies.
  • Attend training and conferences related to the SPF SIG initiative.
  • Function as the day to day coordinator for activities of SPF SIG.
  • Keep partnerships advised as to trends in other communities as well as assisting in the year-end evaluations and sustainability planning.
  • Facilitate networking opportunities for all systems within the coalition.
  • Implement environmental strategies on a county-wide basis.
  • Collect and maintain updated data and statistical information relevant to the SPF SIG project.
  • Participate in community prevention awareness events.
  • Demonstrate an understanding of the disease process of addiction, the core functions of strategic prevention framework, and apply these appropriately to job duties.

EDUCATIONAL/EXPERIENCE REQUIREMENTS:

  • Bachelors degree from an accredited college or university OR a School Diploma or Equivalency plus 3 years experience in the human service field.
  • Good written and verbal communication skills.
  • Working knowledge of alcohol and other drug-related issues.
  • Self-motivator with good organizational skills.
  • Public speaking experience, preferred.
  • Good at communicating in diverse situations.

CONDITIONS FOR CONTINUED EMPLOYMENT:

Addiction Nurse LPN

Reports to: Program Director / Nursing Director
Location: Women & Children- Sioux CIty, Synergy Center- Cherokee (full time 2-10pm M-F)

  • Monitor and document patient compliance with unit rules.
  • Document each patient’s participation in treatment activities.
  • Communicate with physician and implement orders.
  • Interpretation of diagnostic procedures and lab values with direction from Nursing Supv.
  • Monitor and document patient compliance medication and doctor's orders.
  • Insure patient’s medical needs are addressed.
  • Transport patients as needed.
  • Report out of the ordinary situations to the supervisor or on-call manager.
  • Advocate for patients and families, and promote destigmatization of the disease of addiction.
  • Maintain professional, personal, and emotional balance through continuous self-examination, supervision and feedback.
  • Be flexible to changes in scheduling with adequate notice.
  • Assist Nursing staff in the medical care of patients.
  • Complete vitals on patients as needed.
  • Administer medications.
  • Document medical occurrences in patient record.
  • Assist in medication documentation and organization.
  • Insure nursing orders are followed through.
  • Assist in scheduling and follow through of medical appointments.

  • LPN  License
  • Experience in the addiction/ mental health field (preferred)

  • Nursing groups are documented as performed.
  • Patient medical interventions and treatments are documented as they occur.

 Customer Satisfaction:

  • Patients indicate their medical needs were met when completing the customer satisfaction survey.

Shifts: Full and Part Time, 2pm-12pm,12am- 8am
Location: Sioux City - Adolescent Center

Essential Job Functions:

  • Establish working relationships with patients and families.
  • Facilitate Recovery groups as assigned.
  • Monitor and direct activities in Learning Laboratory when in use.
  • Monitor and direct patient milieu at all times.
  • Monitor and document patient compliance with unit rules.
  • Document each patient’s participation in treatment activities.
  • Monitor and document appropriate passing of medications.
  • Transport patients as needed in company vehicles.
  • Report out of the ordinary situations to the supervisor or on-call manager.
  • Advocate for patients and families and promote destigmatization of the disease of addiction.
  • Maintain professional, personal, and emotional balance through continuous self-examination, supervision and feedback.
  • Be flexible to changes in scheduling with adequate notice.
  • Be available to take on-call on a rotation schedule. 

Educational/Experience Requirements: 

  • High School Diploma or Equivalency.
  • Experience in the addiction field (preferred).

Essential Job Functions:  

  • Establish therapeutic relationships with patients and families.
  • Facilitate individual, group and family therapy as assigned.
  • Conduct ongoing assessment, treatment planning and discharge of patients.
  • Coordinate services with referral sources and other community resources.
  • Develop and maintain the confidential patient record.
  • Advocate for patients and families, and promote destigmatization of the disease of addiction.
  • Maintain professional, personal, and emotional balance through continuous self-examination, supervision and feedback.

Educational/Experience Requirements:

  • Bachelor'ss Degree in Human Services plus 1 year experience in human services field OR High School Diploma or Equivalency plus state certification in substance abuse counseling OR completion of the JRC Academy Program.
  • Good written and verbal communications skills.
  • Good interpersonal skills.
  • Bi-lingual skills (preferred). 

Conditions For Continued Employment:

  • Maintain CPI certification.
  • Attainment of IBC Certification or Iowa Licensure within 15 months.
  • Attend required trainings.
  • Attendance at Adult/Child Abuse training.
  • Maintain PSO privileges and required CEU’s.
  • Remain open to clinical supervision and opportunities for growth.
  • Meet Jackson attendance guidelines.

Objective Outcomes of Job:   

  • Productivity: Maintain at least 50% personal productivity on a monthly basis.

  • Clinical Service: Average group size of 8.
  • Patient Outcome: WMA % of at least 60%

  • Length of Stay: Maintain an average length of stay determined for each level of care.
